Working Together to Improve Seniors’ Care

On March 6th, the North Simcoe OHT Seniors Services Working Group hosted a process improvement workshop at the Midland Public Library, bringing together approximately 40 patients, caregivers, and providers from across the health, community, and social care sectors.

Co-chaired by Alison Howard (County of Simcoe) and Cheryl Wilson (Independent Living Services), the group is focused on improving access to high-quality, person-centred care for older adults across North Simcoe.

The session explored the current journey of older adults—from an Emergency Department visit through to returning home. Participants worked together to map processes, interactions, and key pain points, building a shared understanding of the current pathway and identifying opportunities for improvement.

Early themes included the need to involve family and caregivers earlier, strengthen proactive supports, improve access and capacity, reduce wait times, and continue building a coordinated, community-wide approach. While this work is just beginning, the session fostered strong collaboration and a shared sense of optimism for enhancing care and wellbeing for seniors in North Simcoe.

Integrated Care for Seniors

Integrated care for seniors means that health and community services work together as one coordinated team, so seniors don’t have to navigate the system on their own.

For older adults, this approach leads to better care, better use of resources, and better quality of life. It also helps seniors stay active, involved, and independent for as long as possible.
